The Government announced that, as of December 1st, 2021, businesses will be authorized to operate at 100% of their capacity, as long as people have the complete vaccination scheme.
If you are considering visiting Costa Rica during the December season, these are the requirements that you must take into account to enter.
Vaccinated People
Requirements:
- Consular Tourist Visa if required
- Complete “Health Pass”, it can be only completed 72 hours before entering the county. View the form on this link
- Attach the Vaccination Record Card in the “Heath Pass”
QR CODE Acquisition
People with digital sign
- Fill out and complete the form enable on the page of Ministerio de Salud for this purpose the form can be found on this link
- Complete the affidavit and digitally sign it
- Copy of the vaccination record card
- If the vaccination record card is in another language, you must present a Spanish translation
- To have completed the Heath Pass when you entered the country after vaccination
Requirements from 2 to 5 must be sent together to the following email: notificacionescovid@misalud.go.cr
People without digital sign
- Fill out and complete the form enable on the page of Ministerio de Salud for this purpose the form can be found on this link
- Complete the affidavit and digitally sign it.
- Copy of the vaccination record card.
- If the vaccination record card is in another language, you must present a Spanish translation.
- To have completed the Heath Pass when you entered the country after vaccination.
Requirements from 2 to 6 must be sent together to the following email: notificacionescovid@misalud.go.cr
Unvaccinated people
Requirements:
- Consular Tourist Visa if required
- Complete “Health Pass”, it can be only completed 72 hours before entering the county. View the form on this link
- Attach the Vaccination Record Card in the “Heath Pass”
- Acquire insurance that covers care for Covid-19. It must be uploaded to the Health Pass to be reviewed and approved by Costa Rican authorities
Important: From December 1st, 2021 to January 7th, 2022, commercial establishments and mass activities can voluntarily request the COVID-19 vaccination QR code.
